Does it ever feel like you are learning to speak a new language when you are
trying to talk about having a stroke? When you are navigating a brand new path
of healing and recovery for yourself or a loved one, the last thing you want to
deal with is figuring out what certain complicated words mean and how they
relate to your situation. Some examples of words that float around when
discussing a stroke diagnosis and recovery may include: hemianopsia, dysphagia,
diplopia, apraxia. It’s enough to make a person tongue-tied! In this post and
future posts, we will break down some of the intimidating words a bit so that
you will have a quick reference guide and feel more empowered to have meaningful
discussions with doctors and therapists. The more prepared you feel to readily
discuss some of these topics, the better you can advocate for yourself or your
loved one.

In today’s post, let’s discuss some words that relate to speech and
communication disorders that can happen when a person has had a stroke.


Aphasia: a communication disorder that happens as a result of an injury to the
brain. It is an impairment that can happen after a stroke, brain injury, brain
tumor, or some types of brain infections -- no one is born with aphasia. Not all
strokes or brain injuries result in aphasia.

Aphasia causes trouble with communication and use of language, which can include
problems with speaking, understanding, reading, and/or writing. It can range
from mild to severe, and can have one or multiple noticeable symptoms. A person
with aphasia may substitute one word for another in a sentence, need extra time
to generate words, use words that are not recognizable, shorten sentences and
lose context, say things that don’t make sense, not speak at all, or not be able
to understand what someone is saying.

There are several other descriptive words that you may hear in discussions about
aphasia. Some of these words describe or label the symptoms of aphasia that
appear to be affecting the person the most: expressive aphasia, receptive
aphasia, global aphasia. Some of the words indicate the area of the brain that
appear to be most impacted: Wernicke’s aphasia, Broca’s aphasia. After carefully
reviewing medical records and completing a thorough evaluation, the speech
therapist involved in treating the person’s aphasia will help explain which of
these words are most closely linked to the specific case and how they can best
be managed.

Aphasia does not affect or change a person’s intelligence.

The main treatment for aphasia is speech therapy. Therapy can provide intensive
training to relearn communication skills, as well as teach a person how to use
adaptive strategies to communicate despite the disorder. Some people are able to
learn to use gestures, write short messages out, use a communication aide such
as a tablet or picture board, or rely on “yes” and “no” answers in order to
communicate. Others may need visual or sensory cues to understand a message,
such as a demonstration of a task or tapping on a body part to signal using it
for a task. Some may struggle to portray or understand a message at all, while
others may return to their baseline, or get pretty close to it!

Dysarthria: a different type of communication disorder that affects the
pronunciation and clarity of words. “Dysarthria” is a word that describes a
condition when a person’s speaking muscles (those that help the lips, tongue,
cheeks, throat, and jaw do their jobs) are weak or uncoordinated. Dysarthria is
another condition that occurs related to brain damage from things like stroke,
brain injury, and brain tumors, but it does not happen in every case.

When a person has dysarthria, words may come out sounding slurred, rushed,
muffled, or jumbled. It may sound like the person is speaking with food in his
mouth, or is talking too quickly to pronounce things accurately. Sometimes the
person’s appearance of drooping face muscles on one side may cue you to realize
why she is speaking differently, while in other cases a physical feature may not
be obvious.

The main treatment for dysarthria is also speech therapy. Your skilled clinician
will coach you on strengthening and retraining your muscles, teach you
strategies to slow down and speak more clearly, and help you pronounce words so
that your wants and needs can be more accurately verbalized and understood by
others.

There are things you can do to help a person with aphasia and/or dysarthria feel
better and safer about trying to communicate. Try to speak slowly and clearly,
wait for an attempted response, and do not “talk down” or patronize. Help the
person know that you understand his intelligence level is no different than
before. Minimize distractions in the environment so the person can concentrate
on listening only to what you have to say, and on trying to respond the best she
can. Do not answer for the person or assume you know what he is trying to say.
You may need to gently ask for the person to repeat herself, speak more slowly,
or clarify her point. Most of all, try to be patient. A person with aphasia or
dysarthria still wants to be able to communicate with you in any way he or she
can.

A “healthy lifestyle” can really take on a lot of different meanings depending
on who you are talking to! It can sometimes seem overwhelming and hard to think
about getting started with being healthier, especially with the added pressure
of either healing from or trying to prevent a stroke. But I’m here to tell you
that you can get started today on making small changes that can have big,
lasting impacts on your health and wellbeing, and hopefully help you recover
from your stroke or prevent one in the future!

There are several key factors

to improve your cardiovascular and brain health and healing. My advice to help
with getting over the short term hump and build the change into a more
successful longer term plan is to choose one thing you can get started with
today… then build from there. Set a manageable goal (that you can measure
somehow!) Check in with yourself, or ask someone to hold you accountable. Make
the change fun in some way, so you can look forward to it instead of dread it.

Celebrate victories, and give yourself grace with days that are harder to stick
to the plan

We are all human and change is hard. Here are some ways to get healthy!

1) Eat better.


Try not to let this scare you! With all of the trends and fads, changing
guidelines, and amount of information out there, it may seem daunting. Instead
of thinking you need to do a total overhaul of your eating, pick something
doable. Some ideas to get started (again, pick one or two to start with):

-Buy foods with less sodium and don’t add extra salt to your food.
-Eat more fruits and veggies. Eat what you normally eat, but add a small helping
of a fruit or vegetable to each meal.
-Don’t eat as much sugar (I didn’t say to take it all away!) Switch out a bottle
of water for your soda. Have dessert every other day instead of every day.
-Try to cook/prepare more meals at home instead of eating out to avoid some of
the fried choices and added sugars.
-Choose foods that have the Heart-Check symbol (this means they are deemed
heart-healthy by the American Heart Association!)

2) Be more active.
Again, try not to let this seem too hard. Start by taking a short walk once a
day, and gradually add to your distance. Take a new fitness class. Try to get up
and move around more instead of remaining seated. Adding resistance (weights)
benefits your muscles (and your heart is a muscle!) but again, try something
small to start with so that you can be successful. Add on to the amount of time,
resistance, or distance gradually so you can increase your intensity for even
better care of your heart and brain.

3) Stop Smoking.
Easier said than done, I know! Work with your primary care doctor to figure out
a truly feasible strategy, and stick to it. Remember that you are trying to let
your brain heal (or that you are trying to avoid a stroke). Smoking increases
your risk of heart disease and stroke. You can do it!

4) Manage your blood pressure

Set up an appointment with your primary care doctor today. Get all of your
important labs checked, and see what your doctor comes up with for a plan. If
this includes medications, make sure you understand how and when to take them,
how you will get your prescriptions, and how you will make sure to remember to
take them (that may mean using pill box organizers and putting your medications
in front of the coffee pot so you’ll see them each morning!) Some parts of the
strategy may be related to the above topics (eat better and be more active).
Collaborate with your doctor and advocate for yourself to figure out a plan that
you can stick to. Blood pressure, cholesterol levels, and blood sugar can all
impact your stroke and heart disease risk if they get too far out of the safe
range for too long.

5) Address your mental health.

Check in with yourself each week: are you getting enough sleep? If not, see
about making small changes such as putting your devices down at least 30 minutes
before bedtime, or setting up a more peaceful sleep environment (dark, quiet, no
TVs). Try to work on stress management, such as saying no or delegating tasks
when you have too much going on. Recovery (and prevention) are important, so
remember saying no is okay! Make sure to prioritize yourself and put self-care
high on your list. If you are seeing an occupational therapist, the OT can help
with sleep hygiene, suggesting calming strategies, and help you find a way to
make this more routine in your life.

And don't forget: you can do this!
Empower yourself and be your own best advocate. Make a plan that you can stick
to! This often means choosing one particular thing to start with, and sticking
with it until it becomes more routine for you. Usually this takes 2-3 weeks, so
don’t give up too quickly. This is your personal journey to better health, brain
healing, and prevention. Only you can decide how to best get yourself on board
with these key lifestyle changes so that you can stick to your healthier
lifestyle journey. You’ve got this!

Managing Arm Swelling Following a Stroke

frustrating consequence: swelling in the affected arm and hand. In a
normally-functioning arm, muscle and joint movement pump lymph (tissue fluid)
throughout the body. Weakened arm muscles from a stroke are less able to pump,
causing fluid to accumulate towards the bottom of the limb. This fluid buildup
results in arm swelling, which is also known as edema. Early recognition and
treatment of edema is vital to prevent pain and improve arm function. (1, 2)

Strategies for managing post-stroke arm swelling:
 1. Elevation
 2. Compression
 3. Massage
 4. Exercise
 5. Electrical stimulation

 Elevation
“Dependent” or downward arm positioning can contribute to edema, as gravity will
pull tissue fluid to the lowest point of a weakened limb. Elevating the limb
throughout the day can help reduce fluid pooling and associated swelling. When
in bed at night, prop up your arm with pillows, ideally above the level of your
heart. During the day, you can try to keep your arm higher than your lap by
supporting it with pillows, a padded chair armrest, or with an arm trough or lap
tray if you are a wheelchair user.



 Compression
Compression garments for the hand and arm may help push tissue fluid from the
bottom of the limb back into bodily circulation. There are various types of
compression gloves and sleeves that can be considered for edema. It is best to
seek advice from a healthcare professional before purchasing a compression
garment, as they often need to be sized for your body to work correctly.

 Massage
Edema massage can assist with fluid circulation throughout the affected arm. It
is best to consult with a therapist or nurse who is also a certified lymphedema
specialist to teach you to perform this technique, as it involves specific
pressure, speed, and direction of massage strokes on the swollen limb. Massage
is contraindicated with some health conditions, and should not be performed if
you have blood clots, heart or kidney failure, or certain cancers.



 Exercise
Exercise is an excellent way to manage edema. Whether you can move the affected
arm by itself, or you have to move it with the help of your strong arm, getting
your muscles to shorten and lengthen can help reduce swelling. Depending on how
strong your affected arm and hand are, you may chose to complete active
movement
exercises

with or without additional weights or passive stretches

techniques can pump pooled tissue fluid back towards the heart. Exercise also
helps maintain joint flexibility and can improve overall strength and arm

 Electrical Stimulation
Your occupational or physical therapist may perform electrical stimulation
(e-stim)

reduce hand swelling following a stroke. This treatment modality involves
application of electrode pads over the skin for the targeted transmission of an
electrical current. There are many types and purposes of e-stim; however,

circulation or aid in muscle pumping to move excess tissue fluid throughout the

Watch for out for dangerous swelling

What is Left Neglect and How Can I Treat it?



What is left neglect and who does it affect?
Many stroke survivors

experience left sided neglect. Some may experience it more extreme than others.
Left sided neglect is defined as impaired awareness of stimuli on one side of
the body, despite not having any sensory loss to that side of the body. Left
sided neglect is a common condition after sustaining an injury to the right
hemisphere of the brain

neglect is a common side affect of a cerebral infarction or haemorrhage.

Does left neglect affect ones independence and safety?
When someone has left sided neglect they may only eat food on the right side of
their plate, put make up on their right side of their face, only thread their
right arm in a sleeve of a shirt, comb their hair only on the ride side or start
writing in the middle of a page. Left sided neglect does not occur because of a
vision problem with the left eye but refers to being unaware of of your affected
side due to damage to the brain on the right side. Left sided neglect can
decrease ones independence and also can be a safety concern.

concern for many reasons. If someone is unaware of their left sided surroundings
they may not be able to cross a busy street independently because they will have
a hard time recognizing cars coming from the left, they may get their left arm
stuck in the wheel of their wheelchair, they may bump in to items on their left
side, they may not see a hot stove that is located on their left side and bump
into it causing them to burn themselves and so on. Since left neglect can impact
ones independence and safety it is very important to address it with your doctor
or therapist.

How does one diagnose left neglect?


Either a Speech-language pathologists, occupational therapists,
neuropsychologists, optometrists, or a neurologists will perform assessments to
determine the severity of the left sided neglect. There are multiple assessments
that can be used. Some examples are providing a sheet of paper with a circle on
it and asking the stroke survivor to draw a clock or giving them a sheet of
paper with a bunch of numbers or letters on it and aksing them to cross out a
specific number or letter. If the stroke survivor is suffering from left sided
neglect many times you will notice them only crossing off letters or numbers on
the right side or drawing the numbers of a clock only on the right side of the
circle.

Is left neglect treatable?
Left neglect is treatable however, there are many different ways one can go
about treating it. The most common way to treat left neglect is forcing the
stroke survivor to pay attention to their left side and to utilize their left
sided extremities on a daily basis. Some examples would be by placing the stroke
survivors night stand on the left side of their bed, placing a clock on the left
side of the room, positioning the TV on the left side of the room or placing
their dinner plate towards their left side. All of these examples force the
stroke survivor to pay attention to their left side.

Constraint induced movement therapy is another way to treat left sided neglect.
Constraint induced movement therapy involves constraining movements of the
unaffected limb with a sling or mitt for 90% of the day forcing the stroke
survivor to use their affected limb for all functional tasks through out the
day. Constraint induced movement therapy is usually practiced in a 2-3 week
span.



There are many ways to treat left sided neglect but all of the treatments stem
from the idea of neuroplasticity

control all of these behaviors and movements by doing repetitive exercises. The
more repetitive exercises and movements one does, the stronger the pathways in

Left Sided Neglect

Stroke survivors are three times more likely to fall at home after having a stroke than the general population. 

Fall Prevention Techniques for Stroke Survivors

million people are treated in emergency departments for fall injuries each year
and over 800,000 patients are admitted to the hospital with an injury secondary

Stroke survivors are three times more likely to fall at home after having a
stroke than the general population. Therefore it is very important for stroke
survivors to understand why they are at a higher risk for falling and how to
prevent a fall.

Why do stroke survivors have a higher chance of falling?

After having a stroke

many people exhibit one or more of these symptoms which can lead to impaired
balance and falls:

• Increased dizziness
• Impaired or loss of vision
• Increased weakness on affected side

What can a stroke survivor do to prevent a fall at home?

1. Increased dizziness can be caused by multiple factors. Dizziness can be
caused by dehydration, side effects from medication and low blood pressure.
Therefore it very important to always stay hydrated, monitor your blood pressure
and always speak to your doctor about possible side effects when starting a new
medication.

2. After surviving a stroke many people will notice a change with their vision.
 It is very important to get examined by an optometrist after surviving a
stroke. The doctor may need to adjust your glasses or they may also recommend
that you speak to an occupational therapist to work on exercises to address your
vision issues. It is important to make sure to always keep your house well lit
and free of obstacles.

3. After experiencing a stroke many people exhibit weakness on one side of their
body which can cause impaired balance. It is very important to continue to work
with a therapist to increase your range of motion and strength on the affected
side. It is also very important to always utilize your assistive device (ie:
walker or cane etc.) if needed.

4. Many stroke survivors tend to have drop foot

Drop foot means that you have a hard time lifting the front part of your foot.
If your doctor or therapist have given you a brace to wear it is very important
that you do so. It is important to keep doing your rehab exercises in order to
strength all the muscles in your foot and hopefully get rid of your drop foot!

5. Many stroke survivors present with impaired cognition after a stroke. Many
people have memory issues causing one to forget to use their assistive device at
all times. It is important that someone with impaired cognition has supervision
at home to prevent falls.  It is also important to set up the home to provide a
successful environment ie clear and light pathways.

How Can I Prevent Having Another Stoke?



According to the CDC [https://www.cdc.gov/stroke/facts.htm], more than 795,000
people in the United States have a stroke every year. About 185,00 of these
strokes - nearly 1 in 4 — happen to people who have had a previous stroke.
Within the first five years after your first stroke, the likelihood of having a
stroke can increase up to 40%. Within 5 years of a stroke, 24% of women and 42%
of men will experience another stroke. On top of that, recurrent strokes have a
much higher rate of death and long term disability. Therefore if you have
suffered a stroke it is very important to make some big life changes in order to
prevent having another more severe one.

Here is a list of a few things you can do in order to prevent another stroke:



1) STOP SMOKING

Tobacco smokers are twice as likely to have a stroke. Tobacco smoke harms your
body in many different ways including thickening your blood, causing blood
clots, narrowing your arteries, and restricting oxygen in your bloodstream.



2) TAKE YOUR MEDICATION AS DIRECTED BY YOUR PHYSICIAN

Many people experience their first stroke due to high blood pressure, high
cholesterol, diabetes or coronary artery disease. People with high blood
pressure for example, are one and half times more likely to have a stroke.
Therefore if you do not take the correct medications to control these issues it
is likely that you will have another stroke.



3) STAY ACTIVE!

Exercise is very important in maintaining a healthy weight and blood pressure.
People who exercise frequently are less likely to have the contributing factors
that lead to stroke.



4) EAT A HEALTHY DIET

Eating foods with lower calories, cholestrol, trans and saturated fats can help
you maintain your weight as well as your cholestrol levels. This can
significantly decrease your chance of having another stroke.



5) DRINK ALCOHOL IN MODERATION

Excessive alcohol consumption can lead to heart disease and increased blood
pressure. Drinking two or more alcoholic beverages is shown to increase the risk
of stroke by up to 50%.



6) KNOW THE WARNING SIGNS OF A STROKE

It is very important to know the warnings signs

a stroke because the faster you can get help the higher your chance of survival
and making a full recovery. The acronym for spotting warning signs of a stroke
is F.A.S.T which stands for:

F - Face drooping

T - Time to call 911

Overall it is very important to identify the factors caused your first stroke
and what the contributing health issues are in order to make changes to your
lifestyle in order to prevent recurrent strokes from occuring. Most of lifestyle
